Dear Volunteer Chief/Director of Operations/EMS Captain,

The EMS study underway by the Genesee Valley Rural Health Network continues. This phase of the project includes the gathering of information directly from the volunteer Chiefs/EMS Captains/Directors of Operations.

We are requesting to meet with you. The purpose of this meeting will be to collect information and discuss with you issues regarding your EMS planning process, operations, relationships with other organizations, quality management, personnel, etc.

We would like to meet with you within the few weeks. This meeting should take approximately 30 minutes to complete. In order to expedite these meetings I have attached a questionnaire on some of the topics we will cover. Please complete the questionnaire prior to our meeting. Each Fire Chief, EMS Captain, and/or Director of Operations has been sent this survey.

As you know, over 500 surveys were recently sent out to EMS volunteers in the region. Many of your volunteers completed these surveys. The attached survey contains some of the same questions that were in the previous survey. We will be analyzing and comparing the responses from both groups.

I will not be involved in this project from now on as I am moving on a new position as the Fire Chief of the City of Canandaigua. From here on Thompson's EMS Chief Technician Everett Ferguson will be overseeing this project.

Dear Chief/Captain/Director of Operations:

Good Morning, I would like to introduce myself. I am Everett Ferguson the Executive Director of OCALS, replacing Ted Halpin who moved into the position of Fire Chief at Canandaigua Fire Department. I have worked for OCALS since 1983 and have numerous years of EMS experience. We have accepted the study for your area.

The study for the Genesee Valley and Ira Davenport/Rural Metro Rural Health Networks continues. Your assistance is necessary to be able to provide the quality information this study requires. The process of data collection has been delayed because the return of the "Chiefs/Captains and Directors of Operation" survey forms and contacts have not been accomplished.

We recognize the schedule problems facing all of us this time of the year. Therefore, we are requesting you respond by mail with a completed survey form specific for your managerial position. Please do not confuse this survey with the one we sent to all of the EMS providers earlier. THIS IS A MANAGEMENT / MANAGERS Survey. If you require a new survey please call Denise Misik or myself at 716/396-6672. Your assistance is required and greatly appreciated. In this regard please return the survey in the enclosed envelope within 5 days.
