EMS Provider Survey

The provider survey was mailed to 526 individuals in the survey area; 361 in the Genesee Valley Rural Health Network Area (Livingston County) and 165 in the Rural Health Community Systems Network (Steuben County).

We would like to thank the individuals who completed the surveys for their time and input. We have held the specific comments and suggestions in confidence and encourage groups, committees, and administrators to evaluate the suggestions based on their merit for improvement to the EMS System.


Dear EMS Volunteer,

The F.F. Thompson Health System in Canandaigua has been selected by the Genesee Valley and Ira Davenport/Noyes/RM Rural Health Networks to conduct an assessment of EMS. The goal of this project is to improve EMS in your community. An integral element of this assessment is the active listening to the EMS volunteers that provide the patient care. You, along with most of your EMS volunteer colleagues, are being asked to complete the following survey. This survey is your opportunity to make EMS work better for your patients.

The survey is made up of a series of questions asking your thoughts regarding EMS locally. Your responses to this survey will be handled in the strictest confidence. No one, other than F.F. Thompson researchers, will have access to the completed questionnaires. Since several other medics in your ambulance service have been sent this questionnaire, your comments cannot be identified. All completed surveys will be compiled to produce insight into EMS locally. Your response to this survey is voluntary, but keep in mind that this is your opportunity to improve EMS. Overall survey results will be available to individuals who complete the survey on time.

Please complete the survey and return in the self addressed stamped envelope by May 10, 1999. Surveys returned late cannot be analyzed. Your feedback is very important to the EMS community. Thank you for your interest and assistance.
